The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services.
Please ask at reception for details.
We endeavour to complete private requests within 21 working days, dependent upon annual leave and sickness. Please ensure all requests handed into reception are accompanied by a signed ‘consent to release information form’ or covering letter signed by the patient. Without this form/letter we are unable to process your request.
All requests are logged onto the system and the most appropriate doctor will then review and complete. Should the patient need to be seen, reception will contact the patient to arrange an appointment.
When the request is completed, we will inform you by telephone or email and an invoice for any chargeable fee will be raised (these fees can be found under ‘Private Fees’). Any appropriate payment will be required upon collection either by cash, card or cheque made payable to ‘Tenbury Surgery’.
